About the awardee
More than 100 million Americans live with medical debt and the United States leads the world in individual medical bankruptcy rates. At the same time, we lead the world in healthcare spending while our fragmented health care system is an adverse outlier in health system performance in key metrics that include (1) producing positive health outcomes, (2) equitable care, (3) access to care and (4) administrative efficiency.
This complex dissonance of interwoven issues is what Dr. George Goshua's lab in applied health decision science targets. The lab does so across topics of efficient diagnosis, affordable individual-level care, and collective formulary, insurance tier, and health system medical decision making, with an initial focus on people living with hematologic and oncologic diseases. The Goshua lab's work has led to increased uptake of wonderful basic science advancements across hospitals and health systems in the United States while transparently flagging concerning opportunity costs. For example, the lab's research was used to (1) justify health system investment into costly upfront diagnostic tests (that improve clinical outcomes and ultimately save costs due to faster diagnostic adjudication), (2) place the monoclonal anti-CD20 rituximab on hospital formulary frontlines for a patient group with high disease-attributable mortality, (3) create a first global pharmaceutical discounting program for anti-VWF therapy. To ensure patient out-of-pocket coverage in the US and in the lower- and middle-income country contexts, the Goshua lab endeavors to roll their work into clinical guidelines: so far doing so for the care of people with diagnosis of iron deficiency, immune thrombocytopenia, and upfront germline testing for individuals diagnosed with colorectal cancer. To further amplify dissemination efforts, the Goshua lab is working on making all of their already transparently available data freely available in the additional form of public-facing interactive dashboards for the benefit of all stakeholder end-users: patients and patient advocacy groups, manufacturers, payers, clinicians and fellow researchers.
